The check. To change. Control. The Cholesterol™ Gold award recognizes practices that appropriately manage with statin therapy at least 70% of their adult patients at high risk for atherosclerotic cardiovascular disease.
Target: BP™ is a national initiative created by the American Heart Association and the American Medical Association in response to the high prevalence of uncontrolled blood pressure (BP). Target: BP helps healthcare organizations and healthcare teams, at no cost, improve BP control rates through an evidence-based quality improvement program and recognizes organizations that commit to improving BP control. The American Heart Association created the check. To change. Control. Cholesterol™ Initiative with national support from Amgen to improve awareness, detection and management of high cholesterol for consumers, patients and healthcare professionals.
